Driver and owner-operator terms and conditions

This Agreement is between Revival Express Inc. (“Carrier”) and the undersigned Driver or Owner-Operator (“Contractor”).

Carrier operates under authority granted by the Federal Motor Carrier Safety Administration (FMCSA).

By signing this Agreement, Driver/Owner-Operator agrees to the following terms:

1. COMPLIANCE WITH LAWS

Driver/Owner-Operator agrees to:

  1. Maintain a valid CDL and medical certificate
  2. Comply with FMCSA regulations
  3. Follow Hours of Service (HOS) rules
  4. Submit to DOT-required drug and alcohol testing
  5. Maintain a safe driving recor
2. PERSONAL INFORMATION COLLECTION & USE

A. Information We Collect

Revival Express Inc. may collect:

  1. Full legal name
  2. Date of birth
  3. Social Security Number SSN
  4. EIN
  5. Driver's license and CDL information
  6. Medical card information
  7. Driving history (MVR)
  8. Employment history
  9. Background check information
  10. Drug & alcohol test results
  11. Bank account details (for direct deposit)
  12. Contact information (phone, email, address)
  13. ELD and GPS tracking data
  14. Safety and camera footage (if installed in equipment)

B. How We Use Personal Information

Driver/Owner-Operator understands and agrees that Revival Express Inc. may use personal information for the following purposes:

  1. Employment verification and qualification
  2. Compliance with DOT and FMCSA regulations
  3. Safety monitoring and risk management
  4. Payroll and settlement processing
  5. Insurance underwriting and claims
  6. Background checks and MVR reviews
  7. Drug and alcohol testing compliance
  8. Internal recordkeeping
  9. Accident investigation
  10. Government reporting when legally required
  11. Personal data will not be sold to third parties

C. Information Sharing

Information may be shared only with:

  1. Insurance providers
  2. Drug & alcohol consortiums
  3. Background screening companies
  4. Government agencies when required
  5. Brokers or shippers if legally required
  6. Payroll processing services
  7. All sharing is limited to legitimate business purposes.

D. Data Protection

Revival Express Inc. agrees to:

  1. Store records securely
  2. Limit access to authorized personnel only
  3. Protect sensitive data from unauthorized access
  4. Retain records according to legal requirements
3. DRIVER & OWNER-OPERATOR CONSENT

By signing this Agreement, Driver/Owner-Operator:

  1. Authorizes Carrier to obtain MVR reports
  2. Authorizes background checks
  3. Authorizes drug and alcohol testing
  4. Consents to GPS/ELD tracking in company-dispatched equipment
  5. Consents to in-cab camera monitoring if installed
  6. Confirms all information provided is true and accurate
  7. Providing false information is grounds for termination
4. ELECTRONIC MONITORING POLICY

Driver acknowledges that:

  1. Trucks may be equipped with ELD devices
  2. GPS tracking may be active at all times
  3. Dash cameras may record video/audio
  4. Data may be reviewed for safety, compliance, or investigations
  5. Monitoring is for safety and regulatory compliance only
5. OWNER-OPERATOR ADDITIONAL AGREEMENT

Owner-Operator agrees that:

  1. They operate as an independent contractor
  2. They are responsible for taxes and benefits
  3. They must maintain required insurance coverage
  4. They authorize Carrier to verify insurance and compliance status
6. CONFIDENTIALITY

Driver/Owner-Operator agrees not to disclose:

  1. Customer information
  2. Freight rates
  3. Operational procedures
  4. Business relationships
  5. During contract and after termination.
7. TERMINATION

This Agreement may be terminated:

  1. By either party with written notice
  2. Immediately for safety violations, failed drug tests, fraud, or misconduct
8. ACKNOWLEDGMENT

By signing below, Driver/Owner-Operator confirms:

  1. They have read and understood these Terms
  2. They agree to comply with all policies
  3. They consent to the collection and use of personal information as described
  4. They understand violation may result in termination
